Printed on the box is: 115-120V, PH/750T12/5, DEC, D.C. Med Ring Base Up, 750W T12 C13D FILA Clear This item has been in stock in an Electronics Parts store too long and must be sold. It is new in the box. This is a non-returnable lamp which is standard in the industry. If it does not work as brand new we will send a return label and inspect the bulb. If it has never been on it's easy to tell. If the bulb has flashed from too much voltage or if the element has melted from going past it's life span this is not returnable for refund or exchange.
